• Enter the promo code EWPERKS
• Choose what membership plan to avail, click Enroll Now
• Provide the required information, click Continue to payment
• Complete payment using an EastWest credit card

5. Cardholder can use his/her card to avail of the offer for another person.
6. Membership is non-transferable. Use of membership is not applicable to non-emergency cases such as patient transfers.
7. Lifeline 16-911’s Membership Terms and conditions shall apply. Activation of membership application shall take 24 hours from time of registration.
8. The offer is non-transferable and is not convertible to cash or gift certificates.

11. All questions or disputes regarding the Cardholder's eligibility for the promo shall be resolved by EastWest. In case of disputes relating to the promo, the decision of EastWest and Lifeline 16-911 shall be conclusive with approval of DTI.
